Daily Records
- A daily record of the number of half pints of milk served to children. Split cartons of milk may be claimed. (In this event, the milk claimed will be less than the number of children served.)
- An accounting of milk sold to adults and used in cooking. Milk sold to adults or used in cooking may not be claimed for reimbursement.
- Daily HACCP and Food Safety Documentation.
Daily Record Sheet
- Requirements for the daily record sheet depend on the pricing option selected.
- For pricing options 1 & 3, a tally sheet of how many milks served would be adequate.
- For pricing option 2, an attendance roll of the students indicating which students took a milk is required.
- After submitting your Daily Record Sheet, make sure to check off below that you have completed this course requirement.

Financial Records
- Invoices or billings for milk purchased to justify the amount of milk claimed for reimbursement. Maintain the cost of non-milk items (straws, napkins, cups, etc.) separately.
- Documentation for income accrued from child and adult milk payment as well as federal reimbursement must be kept.
- Maintenance of other costs incurred in the administration and operation of the program is also required.
- A copy of each claim submitted for reimbursement to the state must be kept on file.

Administrative Records
- Maintain a copy of the program application, notification of program approval, and the signed agreements with attachments.
- Sponsor is required to administer a self-evaluation for all operating sites on a yearly basis. One copy of this form must be kept on file and another submitted to the State Office.
- Submit the reimbursement claim by the fourth working day of the month. A copy of each claim submitted for reimbursement must be kept on file.
